1. Anti-Infective Therapy: Your Advance Care Pharmacy / Vital Care clinical staff will be involved from the very beginning of your transition to home care.

Antibiotic, antiviral and antifungal medications treat a wide variety of minor to severe infections. Some patients may need infused anti-infective medications to be administered in the hospital or in the home setting through an intravenous catheter. Treatment is focused on controlling the infection and preventing complications. Duration of therapy may range from days to several months depending on the severity or site of infection.

At Advance Care Pharmacy, our skilled clinical team specializes in treating patients who need long- term or complex anti-infective therapies. With a referral to Advance Care Pharmacy, patients receiving intravenous (IV) antibiotics or other anti-infective medications will receive comprehensive care in coordination with the physician.

Our pharmacy staff will work closely with the patient’s health care providers to complete a comprehensive patient assessment before establishing an effective and appropriate treatment regimen. Our patients receive frequent assessments from our clinicians, in order to reach patient-specific goals and maximize patient satisfaction.

2. Nutrition Support Therapy: Total parenteral nutrition (TPN) and enteral (tube) feedings support adults and children who cannot consume or absorb nutrients through regular food intake.

There are many situations when a person cannot eat any or enough food. A patient’s stomach or bowel may not be functioning properly because of an illness or surgery. Under those conditions, the person must receive nutrition in a different way.

Nutrition can be provided either through a feeding tube (enteral nutrition) or an intravenous tube called a catheter that is inserted directly into the veins (parenteral nutrition or TPN). The amount, type, and route of nutrition is determined on a case-by-case basis for each patient.

The Advance Care Pharmacy / Vital Care, Inc.’s Nutrition Support Team consists of nursing staff, pharmacists and a dietitian. Our team of specialists works closely with your physician, making for an easy patient transition from the hospital to the home. Advance Care Pharmacy aims to provide the best possible care from pediatrics to elderly patients receiving nutritional therapy. Our goal is to offer each patient an improved quality of life, by allowing them to return to a thriving lifestyle

3. Cardiac/Inotropic Therapy: Advance Care Pharmacy provides the high level of personalized care necessary for patients living with heart failure, awaiting transplant services, or receiving palliative care.

Congestive Heart Failure (CHF) is a chronic, progressive condition in which the heart is unable to pump enough blood throughout the body. Cardiac inotropes (milrinone, dobutamine, dopamine) may be prescribed and infused in the home setting for those patients with conditions that affect cardiac health. Advance Care Pharmacy / Vital Care's team of clinicians will work with the patient, caregivers, and the physician's medical team to develop a care plan specific to each individual patient.

Your Advance Care Pharmacy clinical staff will be involved from the very beginning of your transition to home care. The goals of Advance Care Pharmacy / Vital Care's cardiology services are to improve the patient's quality of life and maximize comfort through symptom relief. Our pharmacy staff, under the direction of the physician, is able to work closely with the patient and caregiver.

Our clinicians will provide timely and vital information to the patient's health care team, as we understand the importance of providing ongoing clinical assessments and patient monitoring. The Advance Care Pharmacy / Vital Care team will closely monitor the patient’s status and treatment, provide patient education materials, and answer all questions regarding this therapy.

4. Hydration Therapy: Home IV hydration therapy can lead to a quicker recovery, while also eliminating unnecessary hospitalizations.

Intravenous (IV) hydration therapy can help prevent and treat dehydration, or the excessive loss of water from the body. Various illnesses or surgeries can lead to dehydration, complicating the body's normal functional status. This type of therapy allows sterile water solutions and other necessary nutrients to be infused into a patient's vein, thus restoring the normal fluid balance.

Home IV hydration therapy can lead to a quicker recovery, while also eliminating unnecessary hospitalizations. Advance Care Pharmacy / Vital Care provides patients with hydration therapy, with the goal of restoring fluids and electrolytes to return the patient to good health. Your pharmacy's clinical team and support staff will work closely with the physician to coordinate your home care.

5. Immunotherapy: Advance Care Pharmacy clinicians work closely with your team of health care providers in delivering safe and coordinated immunoglobulin (IgG) therapy.

Immune Globulin (IgG) is a sterile solution of concentrated antibodies extracted from healthy donors which is administered into a vein or under the skin. Immunoglobulin infusion therapy is used to treat disorders of the immune system, disorders of the neurological system, or to boost a patient’s immune response to a serious illness. This type of therapy can also be used to treat immunosuppressed recipients of bone marrow transplants.

Advance Care Pharmacy / Vital Care coordinates IgG therapy with the patient’s physician, local nursing agencies, and family members throughout the entire course of therapy on a case-by-case basis. Our clinical team will monitor treatment, help control side effects, provide patient education materials and be available for any patient questions/issues 24 hours a day, 7 days a week.

As your community’s home infusion pharmacy, Advance Care Pharmacy offers our patients a comfortable way to receive this necessary treatment. With the patient at the center of our work, we strive to ensure high-quality care, satisfaction and patient support.

6. Pain Management Therapy: Advance Care Pharmacy provides pain management treatment with the goals of maximizing comfort and improving the patient’s quality of life.

Pain is a common reason that patients visit their doctors. If pain persists, it not only causes physical disability, but pain also places an emotional, economic, and psychological strain on patients and their families.

Pain management is a common infusion therapy provided to patients in the home or alternate site settings. This therapy can be administered by intravenous, intramuscular, or subcutaneous infusions. Effective pain management provides benefits to the patient through earlier return to activity, shortened hospital stay, decreased hospital admissions, and the subsequent reduction in costs associated with the chronic pain.

While the focus of pain management is on the alleviation of pain and suffering, an underlying goal is to allow the patient to rejoin their normal daily activities. Our pharmacy staff will work closely with your physician to complete a comprehensive patient assessment before establishing an effective treatment regimen that meets the patient's needs. Our patients receive frequent assessments to assure adequate pain control and patient satisfaction.
